Contacting the Palliative Care Service

There are palliative care teams onsite at Concord Hospital and Royal Prince Alfred and Chris O'Brien Lifehouse and a team that supports people in the community.

Palliative care referrals are made when extra attention to worrying symptoms or closer follow up at home is needed.  To be referred to the Palliative Care Team you need a referral letter from your specialist or GP. For more information about our palliative care services phone:
